Knowledge and skills needed as a care coordinator
Care coordinators come from a range of backgrounds and may be working in any area within the health sector.
Having a clinical background is not essential to be an effective care coordinator, but an understanding of chronic disease is vital.
To best help people a care coordinator must be aware of the health ‘landscape’ in which they work, specifically:
- the health services available
- how these services are delivered, and
- how to access those services.
